Dear Clients,
Our goal at the Animal Medical Center is to
provide you with the finest veterinary care and service available. The needs of
our clients changed significantly over the years; therefore, in an attempt to
better meet the needs of our clients, we offer the following payment
options:

Cash

Credit Card--Visa, MasterCard, or
Discover

Checks Note: A $35 service fee will
be charged for all returned checks.

Gift Cards
Payment of Services

Payments are due when services are
rendered

Emergency calls, day admissions, and
hospitalized patients will require an initial payment of half the estimated
quote.

All euthanasia fees will be invoiced at the
time of service or within one week of service. Payment is due within 30
days.

If payment is not received within 30 days,
your account will be considered past due. A Finance charge is applied to all
balances on the first of each month.

When an account is 90 days past due and none
of the above payments options have been made at the clinic, the account will be
turned over to an collection agency.
